Across Devon and Cornwall, a programme of digital work is underway to transform the way we provide services to patients.

A key aspect of this project is making sure that people who provide care have the right information at the right time to make the best possible decisions.

This programme is called the Devon and Cornwall Care Record, and the first organisations taking part will ‘go live’ in the spring.

What is the Devon and Cornwall Care Record?

The Devon and Cornwall Care Record is a new system that brings together patient data from a wide range of clinical systems and presents it as a single view.

It enables authorised health and care staff to see information held by other providers across Devon and Cornwall’s health system – from GP practices and hospitals to hospices and care homes.

Most health and care staff will be able to view the Devon and Cornwall Care Record from a link in their own clinical system, although some staff members will need to log in on a separate portal.

What data will be available in the Care Record?

Over time, the Devon and Cornwall Care Record will include a broad range of health and social care data, including:

  • Health problems
  • Diagnoses
  • Procedures
  • Allergies
  • Vital signs
  • Medications
  • Immunisations
  • Radiology data
  • Clinical correspondence
  • Appointments
  • Physical examinations
  • Family history
  • Health setting visits
  • Social history
  • Social care record
  • Lab Results
  • Photographs and images

How will patients and professionals benefit?

The Devon and Cornwall Care Record is an essential part of wider transformation of health and care across the region.

Having access to patients’ overall health and social care information will help care professionals identify problems more effectively and help improve decision-making.

They’ll be able to see what medication patients are taking, what they’ve taken in the past and if they have any allergies. As well as making treatment safer, this will provide patients with greater continuity of care.

The improved administration will save staff time chasing down information and spare patients the frustration of having to repeat themselves to different care professionals.

Who leads the programme?

The Devon and Cornwall Care Record programme team is made up from multiple professional groups across Devon and Cornwall. The team’s key role is to safely implement the system and the team reports to the wider digital transformation boards in Devon and Cornwall.

We’ll be sharing regular updates with key networks through their meetings and bulletins and all essential information will be available on the programme’s dedicated website www.devonandcornwallcarerecord.nhs.uk

However, if you’d like us to attend one of your regular meetings (for example, a primary care meeting) to provide more details or updates about the programme, please get in touch.

We’ll also be organising bespoke workshops to involve you in aspects of the programme as it develops and engaging the public through Healthwatch and established citizen engagement forums.
